As an agency that supplies healthcare workers, we must comply with The Nurses Agencies Regulations 2002 which requires mandatory information regarding all our healthcare workers. Therefore please take care in completing all the relevant sections of the application form(s).
Applicants called for interview must bring the following:
- Fam Daily Care Application Form
- Equal Opportunities Form
- Healthcare Questionnaire
- CRB check – 5 Years Address Record
- Member Bank Details
- Proof of identity – valid passport
- National Insurance Card
- Driver’s License
- Proof of address – any two utility bills / bank statement
- Written Home office permission to work (if not a British or EU passport holder).
- Letter confirming you are a student
- Three recent passport size photographs.
- Most recent Criminal Records Bureau (CRB) enhanced Disclosure Certificate
- Original certificates for evidence of relevant qualifications and training.
- Records of immunisation or vaccinations
- Any statement of Good Health and Fitness to Work Certificate
- For Nurses – NMC statement of registration
- Any professional indemnity insurance
We require only original documents. For your references please provide the employer’s work address. Sorry, references from friends and relatives cannot be accepted.
